ATTENTION: DD WAIVER PARTICIPANTS MUST LET THE STATE AND THEIR CASE MANAGER KNOW BY THIS FRIDAY, JULY 31ST IF THEY WANT THE SERVICES THEY WERE PREVIOUSLY GETTING
If a DD Waiver participant lost Family Living, Supported Living Services or day services following their SIS assessment, they should have received a letter from the state with a form asking whether the participant wants to return to that living arrangement. That form needs to be filled out and received by the state no later than this Friday, July 31, 2015. There is also a form for the case manager to fill out, noting that the issue has been discussed with them. If a DD Waiver participant wants to return to the living arrangement he or she had before the SIS, it is important that they act and submit this form immediately.
If a DD Waiver participant lost any other services following their SIS evaluation, that participant should have received a letter at the beginning of July. The state will restore any lost service not covered by the family/supported living letter as long as the participant informs their case manager that they want it back no later than July 31, 2015. The participant must discuss the issue with the case manager, and then the case manager has until August 17, 2015, to make the request for additional services to the state. It is important that a DD Waiver participant contact his or her case manager by July 31, 2015, to discuss any services lost as a result of the SIS process.
